Nurse's Hours

Our school nurse, Mrs. Bellelo, RN is in her office from 8:15 am to 3:00 pm Monday, Tuesday, Wednesday, and Friday. Amy Johnson, RN works on Thursday. 

 

All medication forms and medication for the new school year 2018-1019 are due by Thursday, July 26, 2018. 



Our new Health Portal is up and running!!  Everyone should have received an email from www.studentehr.com.  All forms are available for upload and you are able to scan right into the portal!!  Please check it out and spread the good news!!

If you are entering a password and getting a message that it is invalid, please remember the password is case sensitive.  If that doesn't help, or if you never received a password or do not remember it, just click the Forgot password link right below the password box.  Your password will be reset and emailed to you from noreply@studentehr.com.  This function is available 24 hours a day 7 days a week and you will receive the email within ten minutes.  If the email does not arrive, please check your SPAM or Junk folder.
 

 

OTC POLICY for GRADES 6-8th ONLY

 

All Medications, including "Over the Counter" medications given by the school nurse, must be prescribed by a physician.  New orders and medication are required by Louisiana State Law every school year.  6-8 Grade Over the Counter mediation form is provided for your convenience but must be signed by both parent and physicians signature and scanned into the Health Portal.  No other school in the Diocese allows this service, therefore, please help the first aid office continue to provide this service.  A separate form is required for each student.  Without this form, the SAS nurse will not be able to administer any OTC medications for symptoms such as headaches, orthodontia discomfort, and menstrual discomfort.  Parents are always welcome to come to school to administer medications to their children

Immunization Requirements

Please be aware that it is a state requirement to provide the current immunization record upon entering school. The immunization record must have the doctor's signature on it and be presented on or before enrollment. Thank you for your cooperation.
